Patients with schizophrenia have a significantly increased risk for colon cancer and a reduced risk for respiratory cancer compared with people without the condition, say UK researchers, who also found that differential cancer risks are not seen in bipolar disorder patients.

There is conflicting evidence as to whether schizophrenia is associated with increased cancer incidence and mortality. However, Julia Hippisley-Cox, from the University of Nottingham, and colleagues say that such studies have been limited by size, the use of biased populations, and the lack of ability to control for potential confounding factors.

The team therefore examined the risk of six common cancers - breast, colon, rectal, gastroesophageal, prostate, and respiratory - in schizophrenia and bipolar disorder patients using data from the QRESEARCH database of more than 10 million people registered at any time in the past 16 years with 525 general practices.

In all, 454 practices met the inclusion criteria, yielding a total of 4,040,494 patients and providing 18,772,868 person-years of observation. The team identified 40,441 incidence cases of cancer and these were matched with up to five controls per case by age, general practice, and calendar time.

There were 10,535 cases and 50,074 controls identified for breast cancer, along with 5108 cases and 24,458 controls for colon cancer, 3248 cases and 15,552 controls for rectal cancer, 3854 cases and 18,477 controls for gastroesophageal cancer, 10,190 cases and 48,748 controls for prostate cancer, and 7506 cases and 35,981 controls for respiratory cancer.

The proportion of cancer patients with schizophrenia ranged from 0.12% of those with prostate cancer to 0.61% of those with colon cancer, while the proportion with bipolar disorder ranged from 0.15% of those with prostate cancer to 0.43% of those with respiratory cancer.
