Comorbid bipolar disorder has a clinically relevant impact on patients with obsessive-compulsive disorder (OCD), with higher rates of narcissism and antisocial personality disorder in patients with both conditions, Italian study findings indicate.

There have been few studies of the clinical impact of bipolar disorder comorbidity on OCD, and none on the influence of such a comorbidity on the prevalence and pattern of Axis II comorbidity, explain Giuseppe Maina and colleagues from the University of Turin.

For the study, 204 individuals who met the DSM-IV criteria of OCD and had a Yale-Brown Obsessive-Compulsive scale score of at least 16 underwent diagnostic and Axis I evaluation using the Structured Clinical Interview for DSM-IV (SCID) Axis I Disorders questionnaire. Personality status was assessed using the SCID Axis II Disorders questionnaire.

In all, 10.3% of the participants met the DSM-IV criteria for lifetime bipolar diagnosis, with 2.0% diagnosed with bipolar I disorder and 8.3% with bipolar II disorder. In comparison with patients who had bipolar disorder and OCD, those without bipolar disorder were more often men and more frequently had sexual and hoarding obsessions, repeating compulsions, and lifetime comorbid substance use disorders.

However, patients with bipolar disorder and OCD had high prevalences of Cluster A and Cluster B personality disorders in comparison with those who had OCD alone, at 42.9% versus 21.3% and 57.1% versus 29.0%, respectively.

Specifically, OCD patients with bipolar disorder had significantly higher rates of narcissism and antisocial personality disorder than those with OCD alone, at 33.3% versus 6.0% and 28.6% versus 4.4%, respectively.
